Slide Out Problem

We have a 2005 Montana Mountaineer 318 BHS. We started having problems with the slideout during Memorial Day weekend. The large livingroom slide started to twist as it was coming o ut. The left side appeared to come out correctly with the right side twisting as it was coming out. Eventually I had to use a adjustable wrench to turn the slide out bar to get it out completely. On departure, I had to do the same to get it back incompletely. Today I was testing and appear to be having the same problem. Any suggestions?
