Another unfortunate update: I tried the slide pads on the floor. They changed absolutely nothing. Every time I run the slide the gears slip more and more. I put a brand new gear pack in and it jumped teeth every single time it goes in or out. The new gear pack looks like a tooth may break off now after attempting yet again to get this thing into correct alignment.

Additionally, the assembly part to prevent crowning is not working. The tube is starting to crown yet again even with the assembly installed. Something is forcing the inner tube to pull up so hard no matter what bolts I adjust on the slide and I can't figure it out.
