If I remember correctly, most CPAP machines draw too many amps for the size wire used in those extension cords. Better off using an inverter or a lot heavier wire than those cords provide. CPAP machines are too expensive to mess around with supplying low voltage. With 12 volt, bigger wire = better voltage.
