Screwy Slideout

Traveling across Texas and put our slides out when setting up camp. Double slide (dining table and couch) went out and so did my vinyl and carpet. A screw in the exterior corner base of the slide had worked down about 3/4 inch and snagged both. Not a pretty sight. DW not happy. Neither was I. Anybody have a similar issue.
