Pullrite Hitch

Do any Pullrite hitch owners out there have any problem with the 3/4 bolt and jam nuts that adjust and secure the nylon pad loosening up after towing the fifth wheel somewhere?

Mine losens up after each time and is getting to be an anoyance. I believe my problem is due to difficulty of getting a wrench in the recessed area to tighten up the jam nut. An open end wrench slips off it as it is at an angle when tightening it and because of the angle, you can't get a box end wrench on it. Anyone having the same problem and if so, did you find a wrench to fit it? If so, where did you get it? I have thought of taking a standard box end wrench and heating it up and bending an offset in it, but would rather buy something if there was something out there instead of altering a good wrench.
