Thinking about it, the ladder most likely was installed after the windows were. It would be very cumbersome to work arround the ladder trying to install the window as the window is installed from the outside.
So the guy installing the ladder would have no idea or reason to know there is an exit there.
The guy installing the windows would have no reason to know where the latter was going to be.

If we are going to place blame let's look at engineering and QC.

Just saying is all.
