Check, then recheck the U-bolts. When I upgraded the springs on our 343RL HC found almost all of the original 1/2" u-bolts were barely beyond finger tight. I also used 9/16" u-bplts and check the nut torque yearly - and some nuts are always looser then they were set. Loose u-bolts will let the axle wander and twist a little bit in the axle or bolt plate and often cause a failure of the center bolt via sear action and possibly a spring pack failure.
