Do I need this
I got a letter from CA DMV that said I need to renew my Health Questionair form DL546 since I tow a fifth wheel. Has anyone else in California heard of this?

You must have a non-commercial class A license that you got to tow the over 10,000 lb trailer. If that is the case, then yes, you will periodically have to send in a new questionaire.

Yep get them all the time even though I drive trucks. Wife gets them too she got the endorsement non commercial to be able to drive for me when I get tired. The price you pay to live in California I guess I have to keep a valid med cert. every two years for my class a license so it's no big deal to just have to fill out a questionnaire for the D W .

Sounds like you have a trailer endorsement or non commercial class A both of which require the self assessment. I just filled one out since I applied for my non commercial class A.

Yes, I had to fill out the DL546 for my 5th wheel. It has to be filled out and on file once a year. It's only required for a noncommercial class A (as opposed to a DL51 for the commercial class A). The noncommercial class A is required if the 5th wheel has a gross vehicle weight RATING of over 15,000 lbs.
