I have been using a checklist created with Microsoft Excel for many years. It is easily modifiable. I print it out on 8.5 X 11 paper. One side has the checklist the other a simply log with a matching number for each completed checklist.
I use a checklist ONLY for the "take off" as I have never found need for one for the "landing" at campsites.

I use / for one campsite and \ for another in EACH check box. The numbers then relate to the log entry for that campsite on the reverse side.
It seems complicated to explain, but very simple to use
