I think part of the problem is the cap less diesel fill tube. There is an easy fix for that: on Amazon you can get a aluminum green cap that says Diesel on the top. It is magnetized as well, so when you take it off, you can stick it to the side of the truck or the fill door. The cap is around $20.

Sorry that happened to you. I work at a Ford Dealer in the service Dept and we have seen this happen a few times. We also have seen several times where they have put gas in instead of diesel. It is a accident that several insurance companies will cover. Both caps are labeled but I can see how it happens. I am very cautious when fueling and putting in Def fluid. GM has the Def nozzle under the hood but have had problems with where the Def tank is located. The tank hangs down pretty low. No easy solution.

After 40 years of owning diesel trucks, my dear husband put gas in the tank and drove about 25 miles. $4000 + $500 tow. Surprisingly, insurance did cover it. The techs at the dealer said they actually see more diesel in gassers! Due to the difference in filler tube sizes, that combo seems less likely, but people do it. Gas stations having different colored diesel handles seems part of the problem. We have both yellow and green for diesel. BP stations are the worst. I just discussed the DEF issue with him as a reminder....
